POLICY’S. 

CANCELLATION POLICY & CREDIT/DEBIT CARD FOR SCHEDULING
I require a credit card/debit card for scheduling appointments.  Your card WILL NOT be charged unless you no call/no show.  

Please provide at least 24 hours notice if you need to reschedule, cancel an appointment, or reduce your appointment hour.  This gives me enough time to fill this time slot.  If you fail to cancel prior to 24 hours before your appointment, or you do not show up for your appointment, YOU WILL BE CHARGED FOR the full price of the service.  I will also charge $25 for a no call/no show or less than 12 hour notice for my consultations. 

 ARRIVAL TO YOUR APPOINTMENT

When you arrive for your appointment please do not come more than 5 minutes prior to your scheduled appointment time (new clients please arrive 10 minutes early). This allows enough time for a short consultation and a relaxed experience. Time may have to be cut from your treatment in order to keep on schedule. Please arrive alone to your appointment as we are limiting the amount of guests in the building at one time. After you are buzzed into the elevator and before coming into the suite, you will be required to stop at the bathroom to wash your hands. Please provide your own face covering. If you come in without your own, I can provide you with one upon leaving. 

INTAKE FORMS

Once you arrive you will be asked to fill out an updated health history form that will include questions regarding Covid-19 and if you are currently experiencing any symptoms. If you have been in contact with someone who has tested positive for Covid-19 you will be asked to schedule at a later date. During the intake your temperature will be taken with a contact-less thermometer. If you have a temperature over 100 degrees you will be asked to reschedule.  


PERSONAL INFORMATION
I will use your email, phone number, and mailing address for reminders and promotions.  I value your privacy and your information will never be shared with a third party.  Please let me know if you prefer to not receive emails or text messages and I will remove this information from my records.  

 I ask that your children be left with loved ones during your appointment.  I do not have a secure lobby or reception area.  

EXTRACTIONS

I am not providing extractions for the time being.
